For Patients: If our squad treated you and you would like a copy of your record, complete and sign our Patient Request Form and return it with a copy of the required identification as directed on the form.

For attorneys: We are not a HIPAA covered entity, and we provide records in response to attorney requests only under a court order. Please contact the receiving hospital for its records.

For billing records: No billing records exist as we provide free services funded entirely by donations.
